Offense Reporting Instruction Sheet
THIS FORM IS INTENDED FOR LOST OR STOLEN PROPERTY. THIS REPORT SHOULD ONLY BE USED IF YOU HAVE NO SUSPECTS, LEADS, FURTHER INFORMATION, OR PHYSICAL EVIDENCE ASSOCIATED WITH YOUR THEFT. IF YOUR PROPERTY WAS STOLEN FROM YOUR RESIDENCE, CONDO, HOTEL, OR VEHICLE, PLEASE CALL DISPATCH AT (970) 668-8600.
This form may also be used to report Common Fraud Schemes. You may also find more information about Common Fraud Schemes and report them to the FBI Internet Crime Complaint Center at http://www.ic3.gov/default.aspx
The crime you are reporting to the SCSO must have occurred in unincorporated Summit County in order for you to report it to the Summit County Sheriff’s Office.
If your incident occurred within boundaries of a town/city that is located within Summit County, you must file your report with that municipal (town/city) agency.

You may file an official Summit County Sheriff’s Office Offense Report by simply doing the following:
- Complete the offense form in its entirety with as much information as possible. If you do not have all of the required information, you will not be able to complete this process.
- You may email this form back to records@co.summit.co.us. Please call (970) 453-2232 for your case number.
- You may fax this form to (970) 453-7329 Attn: Records
- You may mail this form to PO Box 210, Breckenridge, CO 80424
- An acknowledgement of receipt with your case number will be available in 3-5 business days.
The information that you document on the report you submit to the Summit County Sheriff’s Office is of an official nature, and will be processed by the Summit County Sheriff’s Office as an official Sheriff’s Office criminal offense report.
