Children With Special Needs
The Children with Special Needs program coordinator provides resource coordination to address medical, mental and physical disorders. There is no income qualification and no medical diagnosis required.Resource Coordination includes intake, information and referral, and eligibility determination for services. A coordinator is available to answer questions and to provide consultation and support regarding services, other community resource options and future planning. The coordinator also assists families with the development of their Service and Support Plan and helps to coordinate services chosen by the family.
